Rapid weight loss is a well-known risk factor for gallstone formation. When the body breaks down fat rapidly, the liver secretes extra cholesterol into bile, which can crystallize into stones. Reduced food intake also means the gallbladder contracts less frequently, allowing bile to stagnate and stones to form. This risk exists with any method of rapid weight loss.

Risk increases during periods of active, rapid weight loss — typically the first 6-12 months of treatment. Gallstones may be asymptomatic or can cause sudden, severe pain. Risk persists as long as weight loss is ongoing.
