Tirzepatide follows a straightforward titration in 2.5mg increments every 4 weeks. Week 1-4: 2.5mg once weekly (initiation dose, not therapeutic). Week 5-8: 5mg once weekly. Week 9-12: 7.5mg once weekly. Week 13-16: 10mg once weekly. Week 17-20: 12.5mg once weekly. Week 21+: 15mg once weekly (maximum dose).
The 2.5mg starting dose is purely for GI tolerance and provides minimal therapeutic benefit. Many patients begin to see meaningful effects at the 5mg dose. Not all patients need to reach 15mg -- your provider should evaluate your response at each level.
Finding Your Optimal Dose
Unlike semaglutide where most patients target the maximum dose (2.4mg Wegovy), tirzepatide's wider range means many patients achieve satisfactory results at intermediate doses. Some patients maintain effective weight loss at 7.5mg or 10mg without needing to go higher. Higher doses produce more weight loss on average but also more side effects.
